Fair Share Northgate II Wins Innovations Fund Award

LeadingAge and the NewCourtland Foundation announced that Fair Share Northgate II is among seven recipients of the second round of grants through the Innovations Fund, which seeks to catalyze innovation among nonprofit providers of aging services by funding programs that promise to have a demonstrable impact on residents, clients, families, employees or the broader community.
The seven winners were selected because they presented well-developed proposal ideas with the potential to advance the field of housing plus services and to be replicated in other communities. Through the generosity of LeadingAge members and a matching grant from the NewCourtland Foundation, grants of $25,000 have been awarded to each. “Many Northgate II residents have very complex health and social service needs,” said Marilyn Mock, Director of Social Services at Fair Share Northgate II. “We are thrilled to be an awardee of the Leading Age Innovations grant which will allow us to broaden and strengthen the scope of our wellness project. This funding supports our efforts to improve the physical and mental health outcomes of our residents, better coordinate care with our community partners, decrease resident hospital utilization and serve as a model for other elderly and disabled housing programs.”
Fair Share Northgate II will develop the Bridges to Health program to help address residents’ holistic health and social service needs. Fair Share will collaborate with the Camden Coalition of Healthcare Providers to create a care coordination hub to increase resident access to primary care, improve self-care management abilities, and minimize inappropriate use of health resources. Community partners will offer evidenced-based wellness programming, and a community health worker will encourage resident motivation to make healthy behavior changes. Fair Share will also work with the Catholic Charities of Diocese of Camden to provide onsite counseling services to help address depression and grief and loss associated with high levels of trauma (due to poverty, crime and violence) experienced by residents.
“Several of this year’s grantees are address addressing mental health issues, including depression. This is a growing and challenging area of concern that affordable housing communities are seeing across their resident populations,” said Larry Minnix, LeadingAge’s president and CEO. “Other grantees are helping residents to better manage their health and chronic conditions. We are happy to support the development of creative solutions like these that allow seniors to age safely and affordably, with dignity and respect in the place that they call home.”

Fair Share Northgate II (FSNGII) provides social services and health/wellness care for the low-income seniors, disabled people and families residing in the Northgate II affordable housing development in Camden, NJ which is managed by Fair Share Housing Development (FSHD), a nonprofit affordable housing sponsor-developer.
